or even a pure disregard for them, has resulted in death. As might be expected, extensive attention has been devoted to preventing such problems. Workers are typically required
to perform their duties with strict attention to specific guidelines. These guidelines can be workplace specific or hazard specific. They can be drafted by the company itself but
most often they fall under the umbrella of OSHA regulations designed to protect the health and welfare of workers. One example of a
workplace fatality resulting from poor safety procedures is found in a case involving a painter that was assigned to pain the inside of a large holding tank. While painting
inside the tank the painters spray gun nozzle hit a lamp and broke a sealed beam. This ignited a vapor that was inside the tank. The result was
a flash fire in which the painter was killed. When reviewing this disaster it is important to understand that the type of environment
in which this painter was working is not that uncommon in many workplaces. The problems with holding tanks such as these revolve around the types of materials that have
been stored in them. Obviously, materials such as gasoline, kerosene, paint and diesel fuel would present significant concern regarding the possibility of an explosive environment. While each of
these materials is flammable in its liquid form the real danger presents itself when these materials are allowed to transition into their gaseous form. In that form they are
explosive as the unfortunate painter in the example outlined above discovered. OSHA regulations regarding the storage of these materials are designed, in part, to prevent such incidences as these
