American writer, determined to remain in his small American town rather than be counted amongst the expatriate writers of his time, whom he considered perceived themselves as a "cultural elite".
Williams, on the other hand, remained in his home town in New Jersey, a general medical practitioner with wife and family who was also an author of stories, poems and
literary essays. He referred to
himself as a "dog", sniffing out the details of the local community, its inhabitants and its history, and saw working class Americans as representative of the true America: he disapproved
of the development of urbanisation, capitalism and what he regarded as cultural philistinism. Many of his medical stories reflect the conflict between the old and new worlds of medical science,
and the sometimes reluctant responsibility of the doctor to tear away the veils in order to uncover new knowledge.
As Crawford (1993) points out, science, and especially heroic intervention in medicine, is concerned with the conquest
of nature and the unveiling of secrets; a theme which is well illustrated in The Use of Force. As Johnson (2004) notes, the narrative reflects the elitism of the modern
medical practitioner, and also the social issues surrounding medicine, and particularly public health care.
