Many have tried to explain why something works or does not work , but by far, the twelve step approach seems to be the one that most therapists recommend. At
least, in the larger community, and within professional circles, AA and offshoots of it, are the method of choice after one exits an in-patient program. It is sometimes used as
a single method if in-patient treatment programs are not implemented. It seems that twelve step programs are all that is available for those who are afflicted with drug and alcohol
problems. Of course, one has to question the validity of the programs and the definitions that seem to work against the participants. A professional affiliated with AA recently claimed a
success rate of only between 20 and 25% ("The Second," 2003). Critics put the rate much lower than that. When one goes to AA, it is usually coerced. Even
for formal rehabilitation, there is more often than not an event that leads them there and the vernacular for that in twelve step circles is that the person has "hit
bottom." In other words, the individual is at such a low point in his life, he must then realize that the addiction is the cause of all of his other
problems. While labeling alcoholism as a disease for example has changed the perspective a bit, it is also true that whatever one calls it, addiction is a socially constructed
phenomenon. After all, one might ask, why not alter consciousness so much that it provides to some extent a pleasant effect? It is true that in serious addictive situations,
