Since the mid 20th century, the progression of ideals defined under the 12-step program model has been noted for successes for alcoholics and drug addicts. Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) are two programs that have been maintained for over half a century and have defined some of the most common systems for reducing the impacts of addiction and promoting social change. At the same time, psychologists have proposed the use of behavioral models, including the rational-emotive behavioral model, as a basis for creating meaningful change for individuals who suffer from substance abuse. Comparing these two approaches for a specific population may demonstrate the best way to relate change and determine gains in reducing the negative impacts of substance abuse. The 12-step program model utilized by AA and NA focuses on personal acceptance of limitations and the belief that change can only occur when the substance abuser recognizes their own lack of control. Like the talking cures of men like Sigmund Freud, AA and NA programs are designed to move a person through a progression of self-discovery that is based in the telling of ones own story of addiction (AA, 2002). AA and NA programs are also based on the creation of a community of substance abusers who are empowered to support each other through the process (Johnson, 1993). As a result, the alcoholic or the drug addict is not alone, and creates a web of responsibility towards others through sponsorship. The concept of sponsorship not only provides a basis for outreach, but also supports the maintenance of drug and alcohol free status for the individuals by defining a role within the community.
