Decisions at the administrative level frequently pertain to how technologies necessarily involve resource allocation and, frequently, choosing one technology over another may encompass "devaluing or abandoning other technologies," as well as the "reallocation of resources within health care or between wider sectors of society" (Saarni, et al, 2008, p. 618). On the level of the average practitioner, nursing managers are exploring how they can empower clinicians with the "checks and balances" that are inherent to offering "enhanced care delivery," and this frequently encompasses the use of technology in a "clinical decision support system (CDSS)," which refers to "computerized applications that offer care providers" with the "clinical knowledge and information" needed to enhance patient care" (Bowles, 2004, p. 19). While CDSS technology is still in its infancy, nursing literature indicates that the systems, which support "patient preference-based care planning, critical care pharmacology applications, cancer pain reporting...and cancer pain management," are being currently developed (Bowles, 2004, p. 19). Early versions of CDSSs proved to be unsatisfactory due to the fact that they were not integrated with hospital information systems of the clinical workflow, but new versions are showing substantial improvement over these early versions. Recognizing that the task of designing a CDSS is formidable, the Decision Support Workgroup has developed a web-based workbook that is designed to aid in this task (Bowles, 2004).
