do anything. The report, To Err is Human, from the Institute of Medicine, acted as that wake-up call for many health care facilities, including hospitals (Schillie, 2007). It reported that
as many as 98,000 injuries and deaths occur in hospitals each year due to errors (Schillie, 2007). A survey of quality improvement at hospitals sponsored by the Commonwealth Fund
suggested that while many hospital quality managers believed their quality was better than three years ago, there was significant variance in the effectiveness of the reforms (Gearon and Lorber, 2008).
The survey for the Commonwealth study was sent to 470 hospital quality officers, 87 percent of whom believed care of patients had improved at their institutions (Gearon and Lorber,
2008). Only one-third thought care was at the level it should be, which means a great deal of improvement still needs to be made. This is true despite the fact
that quality improvement is part of most hospitals strategic plan (Gearon and Lorber, 2008). One problem is that a much smaller proportion of physicians are involved in quality improvement efforts
