Alzheimers. The story itself, shaped by Jeremy Levens interpretation of the book, is told in narrative and a series of flashbacks as "Duke" (James Garner) reads notebook entries to an Alzheimers patient Allie (Gena Rowlands) chronicling the enduring love story of Noah Calhoun (Ryan Gosling) and Allie Hamilton (Rachel McAdams), who met at a carnival in Seabrook, North Carolina during the summer of 1940. Noah, who lived with his father Frank (Sam Shepard), worked at a local lumberyard while Allie was vacationing with affluent parents, Anne (Joan Allen) and John (David Thornton). The poignancy with which Duke/Noah reads these notebook entries reveals his personal connection with the tale. When Allie registers no recall of the events he is recalling, Duke resembles a grieving man; his grief is a reaction not only to Allies deteriorating condition but also at her apparent lack of memory regarding the love that changed both of their lives. It is during her fleeting moments of seeming clarity and understanding that sustains Duke, and keeps him reading to her in hopes of a miracle breakthrough. The film is perhaps more easily understood from a sociological perspective, by breaking it down into leaders and groups. Clearly, the leaders are Noah and Allie, who refuse to surrender their cause (love) despite the diversity that frequently forces them to evaluate their chosen path. They were not elected to serve leadership roles; rather they emerged as leaders as a result of circumstances. It is their shared quality of determination that enabled them to confidently lead as they charted their own desirable social course.
