1. Introduction Smoking is a addictive habit, but as tobacco is a legal drug it is one that is difficult
to contain, it may be argued the reason for the problem is the failure to appreciate or detect the problems when the drug first became available (Porter, 1999). The cost
to health service is high, in the UK it is estimated that 2000 premature deaths occur each week as a result of smoking, equating to more that one million death
every year(Lewis, 2005). The potential of a smoker dying of a smoking related condition is one in two (Lewis, 2005). In addition to this before death there is also a
high likelihood that they will suffer form a long term disability (Lewis, 2005). With about 25% of the adult population smoking, biased towards low income groups, this is also
seen as a source of health inequality (Judge et al, 2005). One area which is often targeted as a contact point to start the process of cessation of smoking is
primary care (McEwen, 2004). However it is considered the use of tobacco is an addiction and is treated as such (Judge et al, 2005). However, smokers are not limited
in their addition, those who are addicted to other substances, such as alcohol. For example, it has been noted where there is a history of cocaine use there are likely
to be other addictions such as alcohol and tobacco or other opiates (Shankaran et al, 2004). In one survey of 241 women who had been exposed to cocaine during pregnancy
