This is the case, whether the curious person is a journalist wanting to know more about governmental policy, a young child asking why the police are chasing after someone -
or why someone might have convicted an ethical violation. In some countries, people committing a perceived ethical violation would be shot without any kind of inquiry. But this is not
the case in the U.S. Because people are presumed innocent until proven otherwise, systems of inquiry need to be set up to ask questions and dig out information that either
support or deny a persons innocence. Building a system of inquiry, however, is an important task, as it involves many parts. In this
paper, were being asked to put together a system of inquiry for the U.S. Navy. Though the Navy has a code of ethics, sometimes the system of inquiry comes into
question. This is what happened with the Navys "Operation Tailhook" scandal, which took place in 1991 (Donnelly, 1994). The Tailhook Association, a group of active-duty former Navy aviators, would meet
each year at a three-day convention (Donnelly, 1994). But in 1991, some of the sexual hijinks that occur at the event were perpetrated on unwilling women, one of which was
Navy Lieutenant Paula Coughlin, a helicopter pilot (Donnelly, 1994). Though the officers involved were berated, there really was no system of inquiry to delve into what really happened. The upshot
was criticisms against the Navys original investigation, combined with poor questioning of the suspects involved (Donnelly, 1994). Granted, these days, the Navy
is going overboard to prevent another "Tailhook" from occurring. But a system of inquiry when it comes to an ethics violation is necessary to prevent another, similar, scandal from happening.
