Description Philip is suffering from paranoid schizophrenia, currently in a care institution he is due to be released into the community in line with government policies. Before he can
be released as well as being ready with the condition stabilised with medication a CPA has to be created. Philip has a serious condition and the CPA has been formed
through the four stages of the process; assessment of the social and healthcare needs; the agreement of a care plan which is agreed with Philip and some family members who
are acting as informal carers; the identification of a key worker to oversee the delivery of the care plan, who in this case was a community mental health nurse, but
this may be any of a number of the healthcare professionals and could have been the social worker (Carpenter et al, 2004), and finally the set up of the first
review date, at which point the care plan will be assess to ensure it is meeting the needs of Philip as well as other duties, such as those set out
under section 117 (Department of Health, 2006). This meeting required the input from the different team members who were dealing with the various aspects of care1 as he will need
help with a range of issues including social, medical and personal care. The plan developed was a third level plan; a full
