describes in his text Treating the Poor. Dr. Dumont looks extremely serious and says little as we travel to the tenement apartment of Rosa, a Hispanic patient whom he
has tried to help off-and-on for years. Rosa suffers from bipolar disorder and is currently in a maniac stage. Her oldest daughter Lisa called Dr. Dumont, in tears and frantic.
It is Christmas. Rosa, in a rage, has told Maria and Julio, her youngest children that the expensive toys that they see on TV are not for them, that they
need nothing that this country offers. She has thrown the television out of the apartment, and, amazingly, also chucked the refrigerator, and tried to build a fire in the middle
of the kitchen floor in order to demonstrate self-reliance to her children. When we arrive, the children cower in a corner, terrified of their mothers rage and anger.
Dr. Dumont offers the usual pharmacological remedies. Dr. Dumont has tried over and over again to persuade Rosa to take the Lithium he prescribes on a regular basis. He also
tries to persuade her each time to come into the clinic for counseling and therapy, but Rosa will not adhere to any regime for more than a few visits before
announcing that shes "fine" and then another year or two will pass before the next outburst of psychosis. There is resignation and frustration evident in Dr. Dumonts voice as
he speaks to Rosa. Dr. Dumont has spoken of Rosa and her situation to me before. It is clear that he connects her mental instability to her poverty and
sense of helplessness. She becomes psychotic when she is overwhelmed by guilty, shame, frustration and anger. He has commented on how he sees commitment of the poor, such as Rosa,
