ulcers are painful, and potentially life threatening as they increase the patients risk for infection. A PU is defined as "an area of localized damage to the skin and underlying tissue caused by pressure, shear, friction and/or a combination" of these conditions (OHare, 2008, p. 62). Research evidence indicates that hospitals and nursing homes that have an elevated baseline rate in regards to the incidence of PUs in their facilities can decrease both the incidence and prevalence of PUs by implementing improved practices (Lynn, et al, 2007). The incidence of PUs within a nursing home facility is acknowledged to be an indicator as to the facilitys quality of care (Kayser, et al, 2008). While clinicians evaluate PUs according to the depth of tissue injury and designate them as ranging in severity through several stages of deterioration, publicly reported indicators of nursing home quality combine the incidence of all stages in determination of patient care. This determination is based on the assumption that "higher rates of superficial lesions correlate with higher risks of full-thickness ulcers" (Lynn, et al, 2007, p. 1663). The Centers for Medicare and Medicaid Services (CMS) recruited the "quality improvement organization (QIO) for the state of Washington," which is Qualis Health, for the purpose of leading a "collaborative quality improvement project" that focuses on PUs in nursing homes as its primary focus (Lynn, et al, 2007). QIOs, as well as its corporate partners, recruited nursing homes to participate in a quality initiative entitled the National Nursing Home Improvement Collaborative. This collaborative ultimately involved "29 quality improvement organizations and six multistate long-term care corporations," which recruited "52 nursing homes in 39 states," with the goal of implementing quality improvement strategies (Lynn, et al, 2007, p. 1663).
