a producer of model airplane kits. Quality acceptance for these die castings is based on a visual inspection of parts for appearance. After an audit of the process, the quality
assurance department indicated there was no change to the process, and that the increase in defects involves a variation related to a specific, assignable cause. The president of the company,
uncertain what this means, wants an explanation. What will he be told? The key here is to tell the truth - that some
kind of variation was added to the process which, in turn, created an increase in defects. In such situations, its better to start from the end and work backward and
determine what went wrong and the cause of it. Once that cause has been isolated, it can be corrected. I would also be tempted to go beyond visual inspections for
quality, as such inspections are done by human beings, who make mistakes. Wang, et al (2011) point out, for example, that something as minute as turnover of staff can impact
quality and increase defects (as can training or lack of it). Furthermore, even a very small change in design can lead to defects - and its difficult for manufacturing to
test defects that were created on the design level (Dalgleish, 2006). The president now wants the process to be changed so the number
of defects will be reduced. But if the approach is incorrect, how can he be talked out of it? Unfortunately, the knee-jerk reaction of most CEOs and leaders, when something
goes wrong like this, is to "fix it." And many times, fixing it means a wholesale change in processes and/or methods without knowing the specific facts of why things went
