adopted as well as the way its impact any resulting change in performance is measured. When implementing a strategy to reduce the frequency in falls of patients the tools which will be most useful are those which may be a direct help in reducing falls and the IT systems which may be used to support the initiative and measure the efficacy of the program. Question 1 There are many methodologies which may be considered when assessing the implementation of a QI initiative. Three tools that may help at a practical level are tools that help to assess the risk of a fall so that suitable interventions may be targeted at individuals who are most at risk due to their personal situation, and risks which are present within the environment and may be identified and where possible eliminated or minimised with in order to reduce the potential for fall. The first tool to be considered is the Hendrich II Fall Risk Model. This is a useful tool as it is particularly targeted at using an older population, although it is usually utilised in acute care setting (Gray-Miceli, 2007). However, the tool is also being assessed for utilisation in other populations in other settings (Gray-Miceli, 2007). The aim of this risk model is to identify adults which are most likely to be at risk of a fall, taking into account different factors which may converge to increase risk. It is known that forced him to occur as a result of a multifactorial etiology, therefore a model that is able to assess risk by taking into account different variants that may impact on the potential for a full to take place may be useful.
