Psychoanalytical case formulation can be aided by an analysis that is structure around specific theories. Three of the most relevant of these theories in regard to the case of Mr. F (a 42 year old black male) as it is presented in "DSM III and the Psychodynamic Case Formulation", an article provided by the student, are: the drive theory, the ego conflict theory, and the theory of object relations. Mr. F, a married government worker, is obviously driven by one specific desire. That desire is to prove his masculinity to others and, in fact, to himself. The first clue of this desire that emerges in the article is Mr. Fs fear that he is homosexual, his fear that "he will turn into a fairy". The fear can be traced back to his childhood and several instances when his attempts at discovering his own sexuality were mishandled by adults. In the first instance his biological father caught him playing with himself and threatened him with bodily harm if he continued to do so. This resulted in recurrent dreams in which his improperly directed sexual sensations were represented by failure to defend himself or to make items such as guns work properly, items that are obviously representative of Mr. Fs own genitalia. In another instance Mr. Fs mother chastises him for inappropriate sexual exploration with his sister, a correction which served as yet another slap to Mr. Fs psychosexual development. At the age of 13 another incident, this time with an older boy who performed fellatio on the subject, further warped his sense of self identity. These types of incidents obviously fuel Mr. Fs current need to continually prove his masculinity. He attempts such proof through frequent extramarital affairs ...
