office is a factor in clinical outcome. Introduction
Patient access to health care has numerous implications to health. It is logical to presume that better access equates to improved outcome. For dialysis patients access is a
particularly critical factor given that the procedure has to typically be performed several times a week. Access, of course, can be impacted by a diversity of factors. Whether
or not a patient has insurance or the ability to pay for services independently would be an example of two factors affecting access to health care. Distance from the
patient to the health care provider is a particularly critical factor in determining outcome. In an article published in the October 23, 2007 edition of the "Canadian Medical Association
Journal", authors Marcello Tonelli, Braden Manns, Bruce Culleton, Scott Klarenbach, Brenda Hemmelgarn, Natasha Wiebe and John S. Gill investigate the relationship that exists between where a patient lives and the
location of their attending nephrologists. In less densly populated regions like Canada where this study was based, distance between patient and nephrologists can be a particularly concerning factor in
health care outcome. Methods Tonelli, Manns,
Culleton, et. al. (2007) base their study on 18,722 Canadian patients who first started hemodialysis between 1990 and 2000. The distances between a patients home and their nephrologists office
was calculated and Cox proportional hazards models were used to evaluate the relationship that existed between that distance and the clinical outcome. Clinical outcome in these cases included death
