southwest Virginia, is one of the poorest counties in the state as the poverty rate in this county is 140 percent higher than in the rest of Virginia ("County/City Profile"). The per capita income is, on average, only 53 percent of what other Virginians earn. Furthermore, the percentage of people over the age 25 living in Buchanan County who do not have a high school diploma or its equivalency is 155 percent higher than the rest of the state ("County/City Profile"). The following examination of this populations health needs focuses on the topic of nursing in a primary care surgical clinic that serves primarily Buchanan Countys indigent and uninsured population, including Medicaid and Medicare clients. The services provided at this clinic include evaluation and assessment for patients needing hernia repairs, laparoscopic and open appendectomies, laparoscopic and open cholecystectomies, thyroidectomies, breast biopsies, breast lumpectomy, simple mastectomies, perirectal surgeries along with colonoscopies, wound debriedment, and incision and drainage of abscesses. Various roles and responsibilities of nurses Because of their indigent SES status, these patients need to be functioning as independently as possible at the time of discharge (Lopez-Bushnell, et al 113). Research shows that through the implementation of an assigning surgical cases a registered nurse (RN) as a case manager, these surgeries can proceed more efficiently and effectively, and that case management results in cost savings, as well as improved patient care (Lopez-Bushnell, et al 113).
