Identification of the problem or process to be changed: Falls by hospitalized patients and/or resident of long-term healthcare facilities are a challenge to care that is becoming progressing more demanding as the population ages and the number of elderly and physically, emotionally or mentally impaired patients increases (Jackson and Gleason 37). Approaches that have been used in the past to prevent and/or reduce falls have either been "too controversial, such as restraints" or cost prohibitive, such as employing "companions to observe patients" (Jackson and Gleason 37). The need for change: Nevertheless, statistics on falls indicate the need for change. JCAHO standards: The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) announced in 2005 guidelines in terms of National Patient Safety Goals and these standards stress the importance of preventing falls ("JCAHO" 33). This standards involved assessing and then periodically reassign each patients risk for falling and then addressing these risks ("JCAHO" 33). Benchmarking: St. Clare Hospital in Lakewood, Washington has been identified by the Solucient Institute as one of the top 100 hospitals in the country, which set the benchmarks for success (Jackson and Gleason 37).
