There are numerous standards that must be successfully achieved by the organization. The focus of this report is Standard 4.1, which states there must be "Valid information sources support
decision making and the identification of consumer / patient pare outcomes." More specifically, the report describes the information and data found in Criterion 4.1.1, which states "Consumer/patient health records am
a primary source of information to support consumer/patient care and safety, improving performance and for managing the organisation." This criterion was selected as the place to begin preparing for
accreditation because a patients health records contain the information all treatment is based on. Therefore, it could be considered the most important information the organization collects and monitors. The
report identifies the many requirements for a satisfactory rating for this criterion. Introduction: Description of EQuIP Program EQuIP is an acronym for Evaluation and Quality Improvement Program published
by the Australian Council on Healthcare Standards (ACHS). The EQuIP program assures specific levels of quality are being met in health care facilities. Certification from ACHS is required. ACHS
will send a team of surveyors to this facility in two years. It is important the facility meets the standards for certification at that time. There are a number
