Introduction Dawn Lights, a 21-year-old university student with early onset moderately persistent asthma reported that she was relatively active, used asthma medication on a daily basis, but experienced an increased need for the use of her short-acting beta2 agonist. Recently, Dawns mother called requesting ambulance service because her daughter could not breathe, was using her sambutamol inhaler every few minutes, but was unable to get relief. Dawn was found unable to speak, sweaty and with blue tinged lips, unable to breath effectively. Immediate observations showed that Dawn was using accessory muscles during breathing, that her respiration rate was 30/min, pulse rate 140 beats/min, and oxygen saturation 88%. Other findings included ECG Sinus Tachycardia and Chest auscultation revealed minimal air movement. Upon arrival, Dawn was treated with high flow oxygen (14 L/min using a non-rebreathing mask) and intramuscular adrenaline (0.5 mg). After 5 minutes, a second dose of adrenaline was administered when breathing did not improve. Subsequently, Dawns breathing improved, oxygen saturation went up to 96% and she was able to respond to verbal questions. Additional treatment included the use of salbutmaol (5mg) via a nebulizer, and hospitalization. During the course of collective observational data, Dawns mother reported that she had a respirator tract infection 5 days prior to the event, and that her asthma had become increasing worse over the course of that time.
