Reducing fraud, abuse and waste has been a primary goal of many of the federal governments divisions, departments and agencies over the past several years. Of course military spending and procurement were among the first to come under scrutiny; most recently much of that attention has been trained on Veterans Administration (VA) hospitals. The purpose here is to assess critical factors of VA hospitals payroll and benefits procedures and to recommend positive changes to some of the routine processes. VA Hospital Fiscal Governance Staffing Located throughout the country, VA hospitals have two definite disadvantages in fiscal matters. First is the spiraling cost of health care and increased demand on physicians time, leading many part time physicians to be paid for far more hours than they actually provide at VA medical centers. The second disadvantage is one that other hospitals would be tempted to see as a benefit: they are virtually guaranteed funding at any level they deem necessary. This condition results from their requirement to serve all honorably-discharged veterans, combined with out-of-control general health care costs. The absence of any market forces influence facilitates the development of inefficiencies and helps to support those already present. Richard J. Griffin, the VAs Inspector General, reported to Congress in May 2003 that the VA has been investigating not only cases of outright fraud and embezzlement, but also staffing inefficiencies that in many cases have resulted in poor quality patient care. The investigation that took place throughout much of 2002 reveals problems with the structure by which VA hospitals use part-time physicians associated with medical schools local to individual VA hospitals.
