80-year-old widow living alone who recently was diagnosed as having Type II diabetes. She has been counseled in the changes and activities she needs to do for herself to
manage her diabetes and to forestall developing complications created by it. The purpose here is to develop a treatment plan for Mrs. C. Current Situation
Mrs. C. recently moved into an apartment building, an independent-living facility for seniors. Before moving to the apartment she lived in her own home and enjoyed
gardening, and the small house was not too much for her to keep up with the lawn care and housework help she hired. Her concern was that she could
have problems (i.e., a fall, a heart attack, etc.), be unable to secure help herself and lie unattended perhaps for days. Also, Mrs. C. is a highly social person
and says that though she was friendly with her neighbors, she could go for several days without having any human contact. Mobility is limited to the extent only that
getting out is difficult. It is not impossible by any means, but it was easier for her not to leave the house for several days at a time.
She surveyed all of the independent living facilities in the local area and chose one; her grown children arranged and conducted the move in
response to several falls Mrs. C. suffered but could not get up without help. She has lived in her new apartment for about four months and is quite happy
