medical/surgical floor, Helen had a temperature of 101.4, shallow but regular respiration and blood pressure of 180/98. Lung sounds and labored respiration led to a diagnosis of pneumonia, and
Helen also was dehydrated, had a urinary tract infection and a stage IV pressure ulcer on her coccyx of approximately 4 cm in diameter. Nursing examination also noted that
she had complete passive range of motion in all extremities, and could not comply with requests for specific movements on the side of her body not reported to be
impacted by the hemipharesis. The following nursing diagnoses can be applied to Helens case: Impaired skin integrity; Impaired mobility; Impaired urinary elimination; Bowel incontinence; Ineffective breathing pattern (NANDA, 2010).
Anna produces documentation of her guardianship of Helen and maintained that she wanted to approve of all medications and dietary elements. In addition, Anna sought approval for attendance
from the commune to provide support for Helen in the hospital setting. Some general concerns occurred as a result of the assessment, including the fact that Helens hemipharesis appears
