long and would be considered a multi-faceted form, as there is just about everything in it to properly analyze an employees performance. Sample Form
As mentioned before, this particular appraisal is eight pages long - and this is longer than the standard one-page form that a rating scale typically has. The first two
pages deal with employee responses, asking the employee about his/her liking of the organization, most important achievements during the past year, elements of the job that are most difficult or
easiest, and so on. The section also asks the employee to think about performance improvement, type of work he/she wants to be doing over the next one, two and five
years and the types of skills/training/experience in which the employee would like to participate. Finally, the employee is asked if the objectives he/she set out to achieve in the past
year were, in fact, achieved. Then the employee is asked to rank himself/herself on various attributes and capabilities. The next four pages
are similar, but these are filled out by the appraiser. At the end, both appraiser and employee discuss objectives to help the employee reach his/her competence, as well as setting
out more goals for the next 12 months. Whats interesting about this particular form, however, is that it encompasses the three most
