One would assume that hospitals are places of healing. After all, patients are admitted to hospitals to receive technology in order to improve their health.
Many people who enter hospitals do so to get well. But Hosford (2008) points out that hospitals are more than magical get-well
places. Theyre places that are staffed by people; people that need to respond to "rapidly changing technology, complex medical procedures...and a diverse scope of operations" (Hosford, 2008, p. 9). Hospital
workers also need to take in and manage huge amounts of data on a daily basis (Hosford, 2008). The result is errors in which patient safety is compromised.
A 2000 Institute of Medicine study showed that medical errors in the U.S. caused up to 98,000 deaths per year, while adding $29 billion
each year to medical and health care costs (Hosford, 2008). Aside from the shock that errors could be costing everyone so much, the report ended up focusing attention on the
errors in a hospital -- leading to the Joint Commission on Accreditation of Healthcare Organizations formulating new patient safety accreditation standards in 2003 (Hosford, 2008). A year later, 24 states
had pushed through legislation mandating mandatory medical error reporting (Hosford, 2008). Additionally, and perhaps more importantly, public awareness of medical errors (and how to report them) has risen as well
(Hosford, 2008). A recent study conducted by Hosford demonstrated that a majority of hospital administrators surveyed reported "substantial progress" when it came
to formulating and using medical error management systems (Hosford, 2008). Additionally, medical errors were reduced by 89%, while hospital quality improvement programs were on the rise as well (Hosford, 2008).
