2011). Information technology, such as the use of electronic medical records, provides needed information at the site where it is needed, improving patient outcomes and helping to reduce errors. Research
that has addressed the causes behind medical errors reveals that they frequently result from a confluence of causative factors that may initiate a breakdown of workflow (Hakimzada, et al, 2008).
This problem is often evident within the context of emergency departments (EDs). At this point, the student researching this topic may wish to include a sentence that personalizes the perspective
