offered by the University of Illinois at Chicago is specifically tailored to offer home follow-up case for children, ages 1-18 (PACMP, 2001). This program offers the services of a Registered
Nurse (RN) and a Community Health Advocate, who both work with families and health care providers n a manner that ensures the continuity of care with the goal of managing
asthma symptoms through an "individualized comprehensive education and home care follow-up plan" (PACMP, 2001). If the student researching this topic, like the writer. suffers from asthma, the student may
