Nursing Case Management for stroke victims can encompass a variety of diverse considerations. Case management is not limited to the time that a stroke
victim is hospitalized but, in fact, extends in most cases for the rest of a patients life. The long-term nature of case management for stroke victims is determined by
the fact that strokes are often recurrent and even when they are not they typically inflict injuries that are never fully resolved. Long term care thus becomes one of
the most pressing concerns in regard to care continuum for stroke victims. Case managers should be involved not just in the immediate needs of a particular patient, however, but
also in the political and societal framework that affects the address of those needs for all patients. Thanks to improved medical knowledge and
treatment, stroke victims today often find themselves living considerably longer than stroke victims in their parents generation. With those changes and with the changes in our familial lifestyles we
as a nation are faced with a number of societal complexities which we havent experienced at any other time in our histories. Partially as a result of improved
heath care practices which result in longer life and partially as the result of the movement away from the nuclear family of yesterday, stroke victims encounter a dilemma in terms
of where and how they will be cared for. Their needs include both short-term nursing care in the face of the acute impacts of a stroke and long-term care
