the rest of the universe that surrounds us. NASA has enjoyed unprecedented successes which include Apollo missions which have put people into space and enabled a man to walk on
the moon; the observations made possible by the Hubble space telescope and the Space Shuttle Program; enabling astronauts to conduct scientific experiment while in a reusable space vehicle in outer
space. NASA was once one of the United States national wonders and a reason for the United States to be proud.
But the space program has not been entirely free of disasters. On January 28, 1986 at the Kennedy Space Center in Florida, during the tenth launch of the shuttle Challenger
a terrible disaster occurred. Just 73 seconds into the flight, the Challenger was engulfed in a giant fireball, killing all 7 crewmembers. Then on February 1st, 2003 the
Columbia space shuttle disintegrated as it attempted to return to earth. All seven crew members aboard were lost. Thus, there have been two total losses of United States
Space Shuttles and their entire crews. These two catastrophic events took place 17 years apart. During the extensive investigation that followed the
Columbia disaster, it was determined that a multitude of the problem which had been uncovered during the investigation which took place directly after the Challenger disaster were still a factor
at NASA. The underlying problems at NASA which contributed to both disasters were communication problems between the engineering department and management. These communication problems were the result of
