On November 7, 2007, the Cosco Busan container vessel struck a support of the San Francisco-Oakland Bay Bridge in heavy fog. The allusion created
a 200-foot long tear in the side of the ship, rupturing several fuel tanks and resulting in a 58,000-gallon spill of diesel fuel onto the surface of the San Francisco
Bay. Authorities acted quickly to contain as much of the oil as possible, but not before thousands of gallons oiled birds and fouled the shoreline in five California counties.1
There was a San Francisco Bar Pilot onboard,2 and the captain of the ship was operating with 25 years experience.3 Superficially, it would seem that those in positions
of responsibility did everything "right," or at least according to policy. The purpose here is to assess the risks involved in the incident and to suggest appropriate changes in
process. Harbor navigation is always difficult; the San Francisco Bar Pilots hold that the San Francisco Bay is among the most challenging of
all portage areas, and certainly is Californias most dangerous waters.4 Once onboard a visiting ship, the pilot has total command; the ships captain is reduced to the role of
messenger for the time that the pilot has control of the ship.5 There is no adversarial relationship between pilots and any regular crew of a large cargo ship.
The San Francisco Bar Pilot organization is polite to the extreme, and it seeks to meet its customers needs in a timely manner.6
