are those that are actually made but do not cause harm, there are those that are actually made and result in death or harm, and there are near-misses. In other
words, the medication error was almost made but caught before the wrong medication or the wrong dose was administered. This essay discusses medication errors, some of the causes, and some
of the solutions. Medical Errors Mistakes have different degrees of severity. There are mistakes that result in death or further illness, there are mistakes that do not result in
death or further illness, and there are mistakes that are near-misses, they were caught and did not cause death or further illness. In 2009, there were 788,558 deaths caused by
hospital-acquired infections and medical errors in the U.S. (PR WEB, 2012). That accounts for one-third of all deaths in the country that year. This essay will focus on medication errors.
A medication error is defined by the FDA as a preventable event that involves inappropriate use of medication or that harms the patient when a professional health care provider is
in control of the medication. Worse, not all medication errors are reported. If the wrong medication has reached the patient, they are typically reported but those near misses are not
always reported. One study revealed that only one out of seven adverse events are reported. This included medication errors (Keinle, 2012). A Federal investigation found that in 293 cases that
involved harm to the patient, 40 cases were reported to the hospital managers, the facility investigated 28, and 5 led to changes in hospital procedures (Keinle, 2012). This is a
