factors such as the inability to correctly calculate dosages (Jones, 2009). Recent technology, however, provides considerable promise in minimizing medication errors. So too does greater attention to the
design and implementation of clinical protocol. In some regions the propensity for medication errors can be quite high. In England and Wales, for example, the probability of error is ten
ten percent (Nursing Standard, 2009). The National Patient Safety Agency (NPSA) has reported that in 2007 there were 86,085 drug mistakes reported to the agency by NHS staff (Nursing
Standard, 2009). This was over a thirty percent increase in medication errors between 2006 and 2007 (Nursing Standard, 2009). Medication errors can occur at a number of points
in the clinical process. One study, however, reports that almost sixty percent of errors occur when medicine is being administered (Nursing Standard, 2009). Many of these errors are
preventable if only the appropriate steps are taken. Sometimes the result of a medication error can be inconvenient but sometimes it can be catastrophic. Indeed, in England
and Wales recent medication errors have resulted in thirty-seven deaths (Nursing Standard, 2009). In addition there were a reported sixty-three instances where patients were severely harmed by the errors
(Nursing Standard, 2009). As might be expected, devising ways to intervene to assure that medication errors become a rarity rather than a rather prominent part of the medical
