In most every hospital setting, efforts to avoid medication errors have been put into place, including specific protocols for medication documentation, patient identification and chain of control over medications once dispensed (Barach and Small, 2000). Though some successes have been gained through the use of protocols, many hospitals still struggle with medication administration issues, especially in emergency and short-stay departments. The existing literature suggests that nursing education and adherence to practices through nursing leadership can be beneficial in reducing medication errors and improving outcomes for both nursing staff and patients. Nurses work on the frontline of interactions with physicians and patients and are often responsible for preventing medication errors from occurring. Medication errors alone account for a large number of deaths in the U.S. each year. In this country, medication errors account for approximately 7,000 deaths each year, with a cost resulting from drug-related morbidity and mortality reaching nearly $77 billion a year (Grissinger and Kelly, 2005). Subsequently, the development of departmental focuses on methods to reduce errors and also improve the safety will focus on individual nursing skills and adherence to protocols for safe medication administration. Many hospitals have put into place surveillance technologies as a means of determining protective strategies for their patients that is based on the tracking of services and medication output.
