approach to assessing the statements made by nurses regarding medication errors and the reporting of medication errors in the hospital setting. This qualitative approach included the use of semi-structured interviews
that were transcribed and data collected through the assessment of major themes. Like many quantitative studies, Stetina, Groves & Pafford (2005) provide a review of literature. Their
review outlines the background information about medication errors, including the number of people in the United States who have been impacted by medication errors and the number of deaths that
have resulted from medication errors. Koppel et al. (2005) used the same introductory elements, relating the number of people negatively impacted, but described medication errors as "adverse drug
events" (ADEs). One of the major differences in these two studies is that there is a clear delineation between medication errors and ADEs, based on the fact that some
ADEs can be the result of unknown allergies or other factors that were outside the control of hospital personnel. The qualitative analysis used by Stetina, Groves & Pafford (2005)
is similar to the approach used in one segment of the mixed methodology of Koppel et al. (2005), especially in the identification of major themes from structured interviews. The
quantitative aspect of the research by Koppel et al. provides a very different approach to understanding the issue. The quantitative data retrieved by these researchers assessed error types, reports
of error risk, and hospital generated error data, utilizing an organizational approach that distinguishes different up to 12 error types. Though the data collected in the research by Koppel
et al. (2005) provides some compelling data regarding the nature and perceptions of medication errors and the risk of errors, the data would seem somewhat dry in the absence of
