4) in what could be considered an understatement. Once considered infallible when it came to diagnosis and treatment, the medical profession these days seems to attract its share of criticism
from all quarters, especially when a patients safety is compromised. At one time, a patients death could be attributed to lack of technology or medical knowledge to save the individual.
These days, however, medical error is also factored into a cause of death. McFadden et al (2004), in fact, noted that medical errors are the eighth leading cause of death
in the United States. Furthermore, medical errors cost hospitals in excess of $5 million per institution (McFadden et al, 2004). The most common
types of errors that can be prevented include technical errors, diagnostic errors, prevention failures and drug dosage errors (von Laue et al, 2003).
But are medical errors the result of human error? Or is it the result of organizational design? Zhan et al (2005) among others, believes organizational design contributes greatly to medical
