a history of insulin dependant diabetes, is currently undergoing physiotherapy following a displaced hip, has been diagnosed with multi-infarct dementia. In addition to this Gladys has also been very disruptive
in the ward where she was admitted, she has been confused and forgetful and when the staff have tried to stop her leaving the ward have resulted in aggressive behaviour.
The plans for her discharge indicate that she is going to be unable to manage at home, her daughter, who is her primary carer, also feels she would not
be able to cope with her mother at home. Despite this Gladys wants to return home and is very adamant that this is the right course of action, her level
of discomfort on the ward has resulted in her refusal to eat when she has been prevented from leaving. The development of a care plan needs to take into
consideration al the aspects of Gladys needs, and the framework of single assessment process will allow, with Gladyss permission, for information to be shared between the different professionals that will
be involved in her carer programme, for both physical health and social needs. There are several areas of concern that cross over between the social and the medical areas,
the care plan needs to look at each and determine the way in which these will be dealt with and also determine who will be responsible for the care. As
Gladyss daughter is a care giver she will also need to be involved in the development of the care plan. The care plan identifies the different risks and needs, how
