of care and the nursing team in terms of organization. A clinical pathway is precisely what the name suggests . . . a path to follow. For example, a "patient
pathway" might be provided to a person when he or she is admitted to the hospital. It lists in easily understandable, non-medical terms what the patient can expect on each
day of his/her hospitalization for their particular procedure. The patient can then make notes of their own on various questions or comments that they need to have explained by their
physician or nurse. Typically, the last page of the form contains discharge instructions and is filled out by the RN upon discharge and is sent home with the patient. The
