The face of health care in the United States changed a great deal throughout the 20th century. As the quality of care increased, patients began living longer, and the focus of medicine became more about extending patients lifespans and increasing the overall quality of their life, rather than simply forestalling the transition to death. As a result, doctors were called upon to provide more differentiated care to more people, and the government needed a way to finance the health costs of individuals such as the elderly who couldnt pay for themselves. This paragraph helps the student address the reasons for the changes in health care over the years, setting the stage for the rest of the paper. Enter the managed care solution. Part of the governments 1970s and 80s strategy to reduce the costs associated with health care, the managed care system forced doctors to bill for procedures on the basis of how much time they spent with patients (Sellaro, 1992). As a result, most doctors began to lose money during this period. Those that managed to keep making money found the nature of their practices changed entirely. One such example is Dr. Louis Mickael. Mickael runs a practice with another doctor, Dr. Charles, sharing office space. Mickael opened this practice in the 1960s and as a result has watched the surrounding community develop over the years from blue-collar workers to upper and middle-management employees as the result of suburban development (Sellaro, 1992).
