consistently show that the health systems has received low levels of public funding. In the 15 years prior to 2000, for example, Indonesia devoted less than 0.5 percent of its
GDP to the health needs of the country (Heywood and Harahap, 2009). A drastic change in the political administration of Indonesia occurred in 1998, after the downfall of President Suharto
and, since 2000, there has been considerable increases in public funding for the health care sector, with health expenditures doubling between 2000 and 2006 (Heywood and Harahap, 2009). Nevertheless, much
remains to be accomplished. The incidence of maternal death in Indonesia has improved greatly over the last twenty years, falling from roughly 650 deaths per 100,000 live births in
1990 to 288 in 2008 (DAmbruoso, 2012). Nevertheless, more needs to done to address this health issue. Based on research, DAmbruoso (2012) indicates that the social health insurance (SHI) system
is in urgent need of reform, and should be designed to extend the reach of the program and improve coverage, as well as knowledge, awareness and utilization among the poor.
A priority should be placed on ensuring that there are a sufficient number of village midwives to attend to the needs of rural women and that these practitioners have the
resources and training required for them to be effective (DAmbruoso, 2012). Governmental policy should encourage midwives to work closely in conjunction with traditional birth attendants (TBAs) as this provides a
