Across the health care industry, spending for information technology (IT) lags virtually every other major industry (Advanced Technologies, 2003). One of the results of this low-end spending is that
systems are diverse and often narrowly-focused, and organizations using them are obliged to provide ongoing and sometimes extensive training. Efforts to contain cost increases in health care have turned
to previously obscure areas, now including IT systems within the health care industry. Foundational Concepts Increased Training In Out of the
Crisis, Deming (1986) described the reasons that organizations should invest heavily in training. Demings (1986) focus was manufacturing, but his Total Quality Management (TQM) - a term that Deming
(1986) himself never used - came to be synonymous with extensive and ongoing training for workers at all levels within the organization. TQM soon was found to have application
in myriad settings in addition to manufacturing. As a philosophy, TQM receives much less direct attention today than it did in the past, but it has become a foundational
concept of businesses of all kinds, including those within the health care industry. Cost Containment The need for efficient operation also is a
matter crucial in todays health care industry. The health maintenance organization (HMO) was born of an effort to reduce the rate of growth in the cost of health care,
and it was able to do just that for several years. Eventually, however, even the HMO concept was not able to sufficiently contain cost increases.
