of the hospitals vision statement in terms of JCAHOs core measure process for heart failure (HF) revealed two breakthrough areas and four breakthrough projects. The HF-l core measure addresses
discharge instructions to HF patients. The breakthrough areas are those of accurate and complete discharge instructions and continuous improvement; one breakthrough project in the discharge instruction area is to
increase JCAHO numerator size. The purpose here is to implement that breakthrough project. The Exit Interview and the JCAHO Numerator Implementation of
this breakthrough project benefits all involved. Of course the patient benefits from the additional information and education s/he receives, which enables the patient to better care for himself after
leaving the hospital. It benefits the patients care givers in that the patient is better able to communicate changes to them and knows more about when s/he should contact
care givers about changes in his condition. The hospital gains benefit in that patient outcome improves and it is able to offer the patient a higher degree of service.
There are six discharge instructions in the JCAHO measure HF-1 that comprise the information the patient needs as s/he leaves the hospital.
They are: 1. "activity level 2. "diet 3. "discharge medications 4. "follow-up appointment 5. "weight monitoring 6. "what to do if symptoms worsen" (Measure Information Form, 2003).
Each of these six points must be included in the patients discharge instructions in order for the exit interview to qualify as a "numerator event" for
JCAHO purposes (Overview, 2002). Each of these six points directly relates to things that the patient can do for himself, with or without the assistance of his family.
