as: voluntary agreement by an individual to a proposed procedure, given after appropriate and reliable information about the procedure, including the potential risks and benefits, has been
conveyed to the individual (AIH, 2008, p. 12). Legally valid consent entails that the person giving consent has the intellectual capacity to comprehend the given information; does so
voluntarily; is given only the vaccines that have been explained and discussed; and has the opportunity to seek further information if desired (AIH, 2008).
If no contraindications to the vaccine are indicated, consent should be obtained for each vaccination (AIH, 2008). Parents or legal guardians have the authority to consent to
vaccination for a minor, which is someone under the age of 18 in all Australian states and territories, except for New South Wales, where adult status is defined as over
age 14; and South Australian and the Northern Territory, where it is defined as over age 16 (AIH, 2008). Documentation following immunization: Immunization policy is established in Australia by
the states and territories rather than the federal government. Currently, six of the eight Australian states and territories require that children should be vaccinated with MMR (measles, mumps, rubella), DTP
(diphtheria, tetanus, pertussis) and polio prior to attending school (Salmon, et al, 2006). This legislation requires "provider-authenticated documentation of immunization status before school entry" (Salmon, et al, 2006, p. 436).
