doctors know is wrong (Rona 2005, p. 87). This was a wake-up call for the healthcare professionals as well as the public. Rona (2005) compares health care to the
Ford Pinto car. Pinto crashes caused an estimated 500 to 900 deaths and reports suggested the manufacturer could have prevented those deaths but they did not fix what they knew
was wrong with the car. They did a cost analysis and it was less expensive to pay off lawsuits than fix the car (Rona 2005, p. 87). Is this what
health care institutions are waiting for? Is it cheaper to have these fatal medical errors than fix the problems? There is a very serious quality gap in healthcare. Many
manufactures use the Six Sigma approach to quality assurance today. The typical accepted measure is 3.4 defects per every million items produced (Rona, 2005). The goal is to reduce even
that small amount. There is no such identified acceptable measure in health care because the only number acceptable for preventable deaths is Zero. Even so, anesthesiologists did focus on this
and they have their error down to just about zero (Rona 2005, p. 87). Different studies indicate that hospitals have about a 97.1 percent rate of success in terms of
errors. That means the error rate is 2.9 per 100,000 patients and this totals about 300 unneeded deaths each and every day (Rona 2005). This is a significantly higher rate
