Working collectively, the individuals and organizations can have a much greater impact on the policy-making process than individuals working alone or even small organizations working on their own (UNC, 2005). Their role is complex and their motives and goals are not always clear. Those involved in the Americans for Disabilities Act of 1990 were quite clear, though. The goal was to have Americans who have disabilities fully participate in American society, including work, education, and society, in general. One of the leading groups with this policy was the National Council on the Handicapped; today, it is known as the National Council on Disability. In 1986, this group prepared a report entitled "Toward Independence", which called for equal opportunity for disabled persons. Prior movements in civil rights provided further support. Thousands of disabled individuals and their families along with local groups lobbied for this bill to be passed (Young, 1997). The Patient Self-Determination Act was also passed in 1990. It required institutions that dealt with Medicare to inform patients that they had a right to have an advance directive for health care interventions. Many health care interest groups recommended this option because health care professionals had found that the end of life is less traumatic for the family when there is an advance directive (Santo, 2010). It is the issue of ending ones life that causes great conflict. The groups most powerful in this debate are religious-oriented groups. The Balanced Budget Act of 1997 included numerous components, some of which dealt directly with Medicare and Medicaid. One group of beneficiaries were low-income children who would be assured health care through Medicaid.
