the past 12 hours, and that pain medication was doing little to relieve the pain. In addition, Paula stated that her pain was in her stomach (abdomen), while her
other noted that her last crisis, which occurred six months prior to this visit, caused pain in the bones of her arms and legs. Paulas history suggests that this is
one in a pattern of crisis that has occurred predominantly over the past year. Paulas mother noted that she was diagnosed with sickle cell disease at 8 months of
age, but that the family history suggested that she might have it prior to the diagnosis. Paulas mother also noted that her pain outbreaks were minimal until Paula reached
the age of 10. After that time, Paula experienced at least one major pain crisis that resulted in hospitalization each year, with the largest number of hospitalizations occurring over
the previous 12 months. A cursory assessment of Paulas developmental, social and neurological functioning suggests that she appears as a normal 12 year old. Paula is slightly smaller
than other girls her age (approximately the 75 %ile), but her mother noted that Paula had not yet begun menstruating, while some of her other friends have. Paulas reflexes,
muscle responses and her muscle strength appeared normal. She complained of pain during assessments of physical condition, but this corresponds with complaints of long bone pain relative to her
condition. Paulas fine motor and gross motor functioning appear normal. At the time of presentation, Paulas mother noted that she experienced abnormal neurological function, specifically "mental confusion" and "lethargy."
