"any system in which the use of health care services is controlled and monitored carefully to ensure that policies are followed, that neither too much nor too little care is
provided, and costs are minimized" (p. 52). In theory, the idea behind managed care is a pretty good one -- controlling health
care services and monitoring them means that people who need care will get it, and just the right amount. No needless X rays or extra tests to worry about, and
no worries about health care going to people who may not require it. It took awhile, however, for the concept of managed
care through Health Maintenance Organizations (HMOs) to take off. And once it did, the horror stories began. In many instances, it was reported that patients were denied care, because the
disease didnt fit the cost-containment formula. Horror stories were told about patients being refused a particular medical procedure, simply because it didnt fit inside a medical formulary. PPOs, or point
of service options, offered a little more flexibility, allowing patients to at least have somewhat more leeway when it came to finding physicians with which they were comfortable. Still, there
were those who didnt like the "gatekeeper" mentality, the fact that any referral or recommendation needed to come from a "primary care physician" before a patient could move forward.
Anecdotal evidence aside, recent literature points out that the managed care model isnt as effective at controlling costs -- or allocating medical care
