twin-to-twin transfusion syndrome (TTTS) (transfusion syndrome, 2004). This is when twins share one placenta with a thin membrane separating them, but each twin in a separate amniotic sac. TTTS occurs when one twin receives more blood than the other twin receives. In other words, the "life-support" system of the placenta favored Karims brother. As a consequence of this, Karim weighted only 1 pound 2 ounces at birth, while his brother weighted 4 pounds 2 ounces. Karim was diagnosed as suffering from severe intra-uterine growth retardation, a hernia and hypospadius. He also suffers from bronco pulmonary displasia, which means that he eats but does not gain weight. Karim was hospitalized for 4 months after birth, while his brother went home after 3 weeks. Karim was tube-fed at first due to reflux vomiting. He was also kept on oxygen to reduce the number of calories expended through the effort that he expends to breathe. At 7 months, Karim receives visits from a nutritionist, a speech therapist, and an occupational therapist. He is an alert and attentive baby. He now weighs around 5 pounds, but eating is still difficult for him. Strategies suggested by the speech therapist help his mother to control vomiting. He must be fed small amounts and then held upright for 15 minutes to allow gravity to help Karim keep the food down. It is also important that his head should be held higher than his body during feeding. When Karim takes a bottle, the sucking tires him and his mother helps by massaging his jaw. The nutritionist has provided special bottles and nipples that also aid Karim. The bottles have 3 lines that aid his mother in keeping the bottle aligned so that the nipple is constantly filled to the maximum, making sucking easier and reducing the risk ...
