diagnosis is the assumption is that there is a link between thought and speech. That is, that since the speech is incoherent by normal standard, the patients thoughts are necessarily
also incoherent. This is something that psychologists, psycholinguists and linguists have been discussing for twenty years (Ribeiro 77). Researcher Branca T. Ribeiro argues that "thought disorders" and "talk
disorders" may have a coherence of their own and she points out that as one analyzes the discourse of a psychotic patient, it becomes clear that the patients speech becomes
segmented into several distinctive frames (Ribeiro 77). Within each frame, the patient "signals a different metamessage," which is indicative of the type of interaction that the patient feels is taking
place at that particular moment (Ribeiro 77). The context of these frames have to do with the prior experiences and expectations of the patient in regards to people, objects and
situations (Ribeiro 77). To substantiate these points, Ribeiro relates her experiences in treating an older Hispanic woman whom she refers to as "Dona Jurema." In response to repetitive
requests to talk, Dona Jurema will alternate between roles?sometime she is the patient, and sometimes the doctor, and sometimes she responds as if she were a child answering her mother
