2011). The Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR) indicates that substance abuse evolves into substance dependence when an individual feels compelled to consume ever-increasing amounts of the substance
over longer periods of time (APA, 2000). Substance dependent individuals typically express their desire to control use, but they are never able to achieve this goal. A diagnosis of Substance
Dependence should include obtaining a comprehensive history form the client, as well as, whenever possible, from additional sources as well, such as medical records and information from a spouse, close
friend or relative (APA, 2000). According to the DSM-IV-TR, alcohol abuse that involves significant interpersonal problems, such as violent arguments with a spouse while intoxicated or child abuse, and is
accompanied by symptoms suggesting "tolerance, withdrawal or compulsive use" should prompt clinicians to consider a diagnosis of Alcohol Dependence rather than Alcohol Abuse (APA, 2000, p. 214). The DSM-IV-TR
utilizes a multidimensional approach to diagnosis that is designed to take into account a broad spectrum of contributing factors that influence and affect a clients mental health ("Psychiatric," 2011). Axis
1 refers to clinical syndromes ("Psychiatric," 2011). As indicated above, Evelyn C. can be diagnosed under Axis I has having a substance-related disorder, which, in this case is Alcohol Dependence.
