Ethical principles guide our actions in the clinical environment. It can be difficult, however, to make all decisions regarding a patients treatment on the basis of ethical
principles alone. Patient care is a complex balance of patient desire, resources, and clinical knowledge. When one counteract the other the therapist is put into a decision of
having to decide on the basis of what is best for all concerned rather than what a patient or their family might think that they want.
Ethical theory, of course, is a critical component of the therapeutic arena. Ethics help us balance out needs, desires, and possibilities. The latter can be
limited not just by our clinical knowledge but also by our clinical resources. Consider, for example, the ethical dilemma that arises when a mentally disturbed patient enters a facility
as a result of a suicide attempt. Obviously, this patient wishes to die. The family of the patient wants the facility to take extra steps to clarify every aspect
of the patients condition and the therapeutic procedure both to the patient and to them. Clinical resources, however, might be limited. The steps that the therapist must take
must be guided by a combination of these considerations. The ethical problems surrounding scenarios such as that outlined above are obvious. One
involves the issue of therapeutic resources. Callahan (1995) emphasizes that as more and more advanced techniques become available in the clinical environment ethical dilemmas become a more obvious aspects
of patient care. Value clarification can help us work through those problems, as can a detailed exploration of the ethical framework that exists in the therapeutic environment.
